Роль кишечного микробиома в патогенезе эндометриоза: механизмы взаимодействия и терапевтические перспективы

Эндометриоз остается одним из наиболее загадочных хронических воспалительных заболеваний репродуктивной системы, и его патогенез до конца не раскрыт. Эстрогензависимый рост эндометриоидных очагов, системное воспаление и нарушения иммунного надзора формируют устойчивый патологический круг, разорвать который существующими терапевтическими подходами удается не всегда. В последние годы все больше внимания привлекает кишечная микробиота как потенциальный регулятор этих процессов. Изменения в составе микробиома способны влиять на иммунный ответ, проницаемость кишечного барьера и метаболизм эстрогенов за счет активности эстроболома и β-глюкуронидазы, формируя условия для гиперэстрогении и поддержания воспаления. В данном обзоре рассматриваются ключевые механизмы участия кишечной микробиоты в патогенезе эндометриоза, включая регуляцию продукции короткоцепочечных жирных кислот, Т-клеточного баланса и гормонального гомеостаза. Отдельное внимание уделяется современным и перспективным стратегиям терапевтической модуляции микробиоты – от диет и пробиотиков до персонализированных подходов, основанных на микробиомном профилировании.
Заключение. Кишечный микробиом следует рассматривать не как второстепенный фактор, а как потенциально значимую мишень в патогенетической модели эндометриоза. Углубление понимания микробиомно-гормонально-иммунных взаимодействий может стать основой для разработки более эффективных и индивидуализированных стратегий лечения этого заболевания в ближайшем
будущем.
