Современные аспекты диагностики и ведения пациенток с «бедным» ответом на гонадотропную овариальную стимуляцию в программах ВРТ.
Пациентки с «бедным» ответом на овариальную стимуляцию (ОС) составляют большую когорту среди всех женщин с бесплодием, обращающихся для проведения программ ВРТ (9–24%). «Болонские» критерии не вполне отражают многообразную группу пациенток с «бедным» ответом на ОС. Новая классификация POSEIDON стратифицирует большую гетерогенную когорту пациенток с «бедным» ответом на 4 группы с учетом возраста пациенток, количества антральных фолликулов и уровня антимюллерова гормона, а также прогноза наступления беременности. Приведены возможные механизмы патогенеза «бедного» ответа на ОС. Особое внимание уделяется пациенткам с нормальными показателями овариального резерва и непрогнозируемым «бедным» ответом на ОС (1,2 POSEIDON), который связывают со снижением чувствительности яичников к гонадотропной стимуляции, обусловленной наличием полиморфизмов генов гонадотропинов и их рецепторов. Обсуждается целесообразность использования новых клинико- лабораторных предикторов «бедного» ответа (FORT, FOI, OSI). Показано, что использование ВРТ-калькулятора целесообразно для прогнозирования числа ооцитов, необходимого для получения хотя бы одной эуплоидной бластоцисты. Представлены различные подходы к оптимизации лечения у женщин с «бедным» ответом в зависимости от принадлежности к определенной группе POSEIDON. Заключение: Использование в клинической практике критериев POSEIDON, уточнение прогностической ценности новых диагностических критериев «бедного» овариального ответа на гонадотропную стимуляцию (FORT, FOI, OSI), а также оценка целесообразности тестирования на полиморфизмы генов гонадотропинов и их рецепторов пациенток с непрогнозируемым «бедным» ответом на ОС могут помочь лучше оценить большую разнородную группу пациенток с «бедным» ответом на ОС и разработать персонифицированные подходы к ОС в программе ЭКО, что требует проведения дальнейших исследований.
