Современные представления о многофакторных механизмах нарушения развития эмбриона in vitro

Остановка развития эмбрионов в условиях in vitro – одна из причин неудач в программах вспомогательных репродуктивных технологий. Около 12–18% пар с бесплодием не получают пригодных бластоцист для криоконсервации и/или переноса на 5–6-е сутки после оплодотворения. Несмотря на весомый вклад хромосомных аномалий, эуплоидные эмбрионы также прекращают свое развитие на стадии дробления. Причины остановки до конца не установлены и активно изучаются. В обзоре рассматриваются возможные патогенетические механизмы остановки развития эмбрионов, включая дефекты активации ооцита, материнско-зиготического перехода, нарушения геномной и эпи­генетической стабильности, метаболическую дисфункцию и клеточный стресс.
Заключение. Многообразие молекулярных патогенетических механизмов, лежащих в основе остановки развития эмбрионов, указывает на необходимость дальнейшего углубленного изучения этого процесса. Расширение знаний о возможных причинах имеет значение как для фундаментального понимания механизмов раннего эмбрионального развития, так и для применения в клинической практике вспомогательных репродуктивных технологий. Выявление ключевых звеньев патогенеза может стать основой для совершенствования подходов к оценке качества эмбрионов, оптимизации условий культивирования и повышения результативности программ экстракорпорального оплодотворения.
